One of the greatest limitations that we experience is the shortage of qualified service providers willing to provide services to the individuals and families that we serve. We appreciate your interest in becoming a qualified provider with Self-Determination Resources, Inc. (SDRI).
If you are interested in providing support services to our customers:
Personal Support Workers:
- If you are not yet a qualified Personal Support Worker (PSW), but have experience working with persons who have developmental disabilities, and would like to become a qualified PSW working with our clients, please contact: [email protected] to request a New Provider packet, and begin the qualification process. We’re happy to help walk you through the process.
- If you are already qualified as a PSW with another CME (Case Management Entity), such as a different Brokerage or a County Developmental Disability Program (CDDP), and wish to also provide services with SDRI customers, we will need you to fill out a small packet of information, which can be picked up in person, or sent to your via e-mail or postal mail. We will need in-person proof (original document) of current photo identification. If you plan to drive our clients and/or receive mileage reimbursement, we will also need a copy of your current automobile insurance. If you have a copy of your current Criminal History Check (CHC) approval, that would also be helpful for us to have a copy on file. If a client you currently serve is transferring from an other CME to SDRI, we may also need you to fill out some paperwork for PPL (Public Partnerships Limited [AKA, PCG Public Partnerships] is the payroll entity that produces and distributes PSW paychecks and direct deposits).

Provider Agencies:
- If you are not yet a qualified/licensed agency through DHS, but wish to become so-licensed, you may wish to visit the DHS Provider Organization Certification page for more information around agency licensing, and/or Contact the licensing office directly.
- If you are already a qualified/licensed agency with a current SPD (Seniors & People with Disabilities) Provider Number in eXPRS, and would like to provide services to our customers, please contact: [email protected]
